Carpal tunnel syndrome (CTS) is caused by entrapment and compression of the median nerve at wrist, within the carpal tunnel, positioned between the transverse carpal ligament and the carpal bones, and causes sensory and motor impairment and pain, especially at night, in the area of the hand innervated by the median nerve.

Eventually, workers with CTS may be forced to give up their livelihood. In one study, nearly half of all patients with CTS changed jobs within 30 months of their diagnosis. Because of the controversy surrounding the issue of carpal tunnel syndrome and workers’ compensation, workers may not always receive compensation payments.

Carpal Tunnel Surgery – Factors that greatly reduce the chances for success:

Having severe symptoms before surgery, such as:

  • Continual numbness.
  • Muscle weakness and wasting.
  • Very poor nerve conduction results.
  • Symptoms that have been present for more than 10 months.

Additional Factors that contribute to the failure of carpal tunnel surgery:

  • Being over 50 years old.
  • Performing heavy manual labor; particularly working with vibrating tools. More than half of people who use vibrating hand-held tools have carpal tunnel surgery failure.
  • Patients who are on hemodialysis have good initial success, but the condition deteriorates in about half of these patients after around a year and a half.
  • Poor mental health or alcohol abuse
  • Patients with diabetes and high blood pressure may be more likely to require a second operation.

Complications after surgery include the following:

  • Nerve damage resulting in constant tingling and numbness
  • Infection
  • Excessive Scarring
  • Pain
  • Stiffness
  • Loss of substantial wrist strength is a complication that affects 10 – 30% of carpal tunnel surgery patients. Some patients who have jobs requiring significant hand and wrist strength may not be able to perform them after surgery. These workers may also have problems in other parts of the upper body, including the elbows and shoulders. These problems do not go away with surgery and can persist. Studies indicate that 10 – 15% of patients must change jobs following carpal tunnel surgery.

If pain and symptoms return, a second carpal tunnel surgery (Revision surgery) will be needed.

Reasons for procedure failure include:

  • Incomplete release of the ligament
  • Extensive scarring
  • Recurrence of the disorder due to underlying medical conditions
